CLAIM FORM

Items notified to us more than seven (7) days after delivery will not be considered. Your claim must be based upon the value of each item at the time of loss or damage, allowance being made for age, wear and tear. You must only claim for the cost of making the item good: you cannot claim a brand-new item. You should obtain traders estimates for repair, where applicable, and send them with the claim form to us within seven days of your claim.

I’ve, being the person (s) named overleaf, declare that the information provided in this claim is true and properly completed to the best of my/our knowledge and belief. I/We understand that if any part of this claim is in any respect fraudulent, all benefits shall be forfeited and actions may be taken against me/us.
